In-home Care For Homebound Adults

Home Health provides health services

to clients that are permanently or temporarily homebound, or when the home is the most appropriate setting if allowed by payer sources. It allows people to remain in their own home and actively participate in their plan of care.

We are fully accredited by the Accreditation Commission of Health Care, and certified by Medicare and Medicaid.

WHAT DOES “HOMEBOUND” MEAN?
You do not have to be bedridden. If you become short of breath, tire easily, need help walking or only leave your home occasionally for medical treatments, you would be considered “homebound”.

WHO IS ELIGIBLE?

  • Persons who live on the Qualla Boundary
  • Must have an order from a physician

HOW DO I GET HOME HEALTH STARTED?
Anyone can phone the Home Health office to request services, but we must get an order from your doctor before we can make a home visit. Sometimes friends or neighbors will call. We still need an order from your doctor, but we will call you first to make sure you are interested in having us visit.

What Do We Do For You?

All services are provided only by order of a licensed physician.
Patients 18 years of age and older are eligible.

Certified Nursing Assistants (CNAs)

  • Assistance with personal hygiene (bath, shower, shave, shampoo, oral care)
  • Nail care (not for patients with diabetes or peripheral vascular disease)
  • Changing drainage bags for urinary catheters
  • Light housekeeping (washing dishes, laundry, mopping floor, preparing meals) along with visits to assist with bathing
  • Medicaid eligible patients may receive aide services under CAP. Level of care is determined by the social worker for the county in which you reside.

Registered Nursing

  • Wound care
  • Insertion of urinary catheters
  • Obtaining lab specimens
  • Administer IV and other medications
  • Teaching use of oxygen, suction, dressing supplies and treatments, specialty mattresses
  • Diabetic teaching
  • Maintenance of enteral feeding tubes
  • Ostomy care
  • Tracheostomy care
  • Teaching disease process, including nutritional requirements

Therapist Services

  • Teaching the use of equipment in the home (such as cane, wheelchair, hospital bed, shower bars, leg brace)
  • Exercises for strengthening and balance, transfer techniques
  • Home assessments for safe environment

Therapist Services

  • Together with Tsali Manor, we offer support to family members who provide care for our Tribal elders
  • Referrals to other community resources
  • Short term respite care based on assessing the needs of the family
