How To Get a Youth Work Permit

  • Applicant and applicant’s employer must fill out the form and return to PHHS Administration building.

  • The Parent/Guardian signature and the Youth’s signature must be signed in front of PHHS staff.
  • Youth ID requirements include their tribal enrollment card.
  • Parent/Guardian ID requirements include two forms of identification, one must be a photo ID.
