How To Get a Youth Work Permit
- Applicant and applicant’s employer must fill out the form and return to PHHS Administration building. Application
- The Parent/Guardian signature and the Youth’s signature must be signed in front of Josephine Teesateskie.
- Youth ID requirements include their tribal enrollment card.
- Parent/Guardian ID requirements include two forms of identification, one must be a photo ID.
